The program aims to improve maternal and child health for families residing in eligible rural areas, with services to be delivered exclusively by licensed nurses implementing HomVEE-approved home visiting models—including, per Amendment 1, Family Connects—while demonstrating model fidelity. Awardees may request up to $100,000 for model fees and training (cap across all grantees), and up to $488,000 for program expansion, with indirect costs capped at 8%. Funded activities cannot supplant current funding and must represent expansion. Required participation includes regional Hub-based networks (Collective Impact sites), CRIS Referral Partner networks, compliance with REDCap data collection (including custom schedules for Family Connects), quarterly three-tiered CQI activities, and annual FCSR and MOVECHS background checks for all relevant staff/volunteers. Grantees must invoice monthly for reimbursement, retain fiscal and program records, comply with audits and site visits, and protect participant privacy under HIPAA and CTF policy. Contracts have a 1-year base with up to four annual renewals, subject to performance and funding. Awardees must register as MissouriBUYS vendors, participate in E-Verify, be current on state taxes, federally compliant, and able to engage in all required reporting and improvement processes.
Eligible applicants are Missouri-based home visiting agencies currently implementing, or working towards implementing, a nurse-delivered, evidence-based, HomVEE-approved model (including Family Connects, per Amendment 1) and able to demonstrate model fidelity. Services must be provided exclusively by licensed nurses, represent expansion/new services, and not supplant existing funding. Agencies must serve eligible rural areas as defined by the RHTP eligibility map. Both nonprofit organizations and governmental entities (with verification) are eligible. Agencies must participate in CTF-assigned networks, CQI processes, REDCap data collection, and complete required background checks for staff or volunteers with direct client contact.
