Rural Health Transformation Funding for Home Visiting

Eligibility

Eligible applicants are Missouri-based home visiting agencies currently implementing, or seeking to implement, a nurse-delivered, evidence-based, HomVEE-approved home visiting model—including Family Connects per Amendment 1—with demonstrated model fidelity. Services must be provided exclusively by licensed nurses, represent program expansion (not supplanting current funds), and serve rural populations as defined by the RHTP eligibility map (https://mydss.mo.gov/media/pdf/rhtp-hub-county-groupings-map). Both nonprofit organizations (with IRS determination) and governmental entities (with documentation) are eligible. Participation in required regional HUB networks, Collective Impact sites, CRIS Referral Partner network, CQI processes, annual FCSR registration/screening, MOVECHS fingerprint checks, and REDCap system reporting is mandatory.

Requirements Checklist

• Completed CTF application (CTF Template)
• Documentation verifying fidelity to a HomVEE-approved, nurse-delivered model (accreditation, registration letter, approved application, training certificates)
• Detailed Program Budget for Year 1 (itemize model fees/training and program funds using the CTF Template)
• Agency Operating Budget (current year, income and expenses)
• Application Certification Letter (CTF Template Attachment)
• Proof of nonprofit status (IRS determination letter) or governmental entity documentation
• IRS Form 990 (if applicable)
• Most recent financial audit summary report
• Organizational chart of leadership and project staff
• Current Board of Directors list with affiliations
• Annual Family Care Safety Registry (FCSR) registration/screening and MOVECHS fingerprint checks for all staff/volunteers with client contact
• Registration with MissouriBUYS and participation in E-Verify
• Compliance with REDCap data reporting, CTF Privacy Policy, audits, CQI/network obligations
