Missouri RHTP: Mobile Integrated Healthcare & Community Paramedicine (MIH-CP) System Expansion — MEMSA Partnership

Summary

The Missouri Department of Social Services, through the Rural Health Transformation Program (RHTP), partnered with the Dept. of Health and Senior Services' Office of Rural Health and Primary Care and the Missouri Emergency Medical Services Association (MEMSA) to expand Mobile Integrated Healthcare and Community Paramedicine (MIH-CP) services across rural Missouri. MEMSA administers the funding directly: rural EMS agencies apply to start up or expand an MIH-CP program or Regional MIH Network, integrating with ToRCH Care Hubs and Regional Coordinating Networks (RCNs).

Eligibility

Rural EMS agency in Missouri collaborating with one or more healthcare partners to start up or expand an MIH-CP program or Regional MIH Network; must integrate within ToRCH Care Hubs and Regional Coordinating Networks (RCNs). Funding limited to entities in Missouri-designated RHT counties.

Requirements Checklist

• Applicants must submit a complete application packet, consisting of: (1) Readiness Assessment, (2) Application Form, (3) Work Plan/Appendix B, (4) Budget Narrative, and (5) Attestation. The application materials must be submitted via email to rhtp@memsa.org. Budget must cover a three-year project period and demonstrate anticipated costs; initial awards are for Year 1 only, with continuation funding for Years 2–5 contingent on federal/state appropriations, satisfactory performance, and continued programmatic need.
