Rural Health Transformation Program (RHTP)

The Rural Health Transformation Program is a federal investment designed to help states strengthen sustainable access to care in rural communities. CMS approves state plans, and each state translates its plan into a different mix of grants, procurements, workforce initiatives, technology projects, technical assistance, and community partnerships. That makes the program valuable but difficult to follow: the evidence is distributed across state portals, agency pages, procurement systems, award notices, and implementation documents. Rural Care Journey brings those public signals together so applicants, vendors, providers, researchers, and program teams can see what is open, what changed, where awards are going, and what implementation work is emerging. RCJ links its records back to public source evidence; CMS remains the authoritative federal source for program policy and official guidance.

Use CMS for federal authority; use RCJ for the working landscape

CMS is the authoritative source for federal RHTP policy, program guidance, and official federal announcements. RCJ complements that source by organizing state-level procurement, award, document, vendor, and implementation activity into a searchable public intelligence layer.
