Proposals must demonstrate innovation, feasibility, sustainability, and impact on rural clinical practice.

Virtual technical assistance for small rural hospitals and rural health clinics transitioning to value-based care and alternative payment models. Applications for the FY27 project period are accepted on a rolling basis from August 15, 2026 through August 13, 2027; cohort selections are announced in Fall 2027, contingent on annual funding approval.

Grants up to $10,000 for organizations located in and serving rural Montana communities. The 2026 cycle opened March 2 and closed September 1, 2026 (first-round decisions June 30, second-round decisions October 9); the program recurs annually, so a new cycle is expected to open in a future year around the same March timeframe.

The Rural Population Health Incubator Program provides grants of up to $10,000 to support population health initiatives in rural Oregon. Eligible applicants include rural hospitals, clinics, public health departments, and community-based nonprofits working to reduce health disparities and improve well-being in underserved communities. The program also offers cohort meetings and specialized support to grantees throughout the grant cycle.
