Michigan Rural Health Partnership Integration and Sustainability - 2027

Summary

Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Funds aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and well being of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships.

Eligibility

Eligible applicants include Michigan-based rural health organizations, federally qualified health centers, local health departments, nonprofit community organizations, hospitals, and clinics serving rural populations. For further eligibility details, refer to the Application Form.
