The grantee will serve as the subrecipient to establish a baseline that can be used to establish targets and demonstrate CHW-related outcomes. The funds will support coordinated outreach, data collection, analysis, and reporting necessary to produce a reliable workforce inventory and needs assessment, ensuring efficient use of resources while avoiding duplication of existing efforts.
Eligible applicants are Federally Qualified Health Centers (FQHCs) serving rural populations in Michigan.
