The grantee will serve as the subrecipient to establish a baseline that can be used to establish targets and demonstrate CHW-related outcomes. The funds will support coordinated outreach, data collection, analysis, and reporting necessary to produce a reliable workforce inventory and needs assessment, ensuring efficient use of resources while avoiding duplication of existing efforts.
Eligible applicants are Michigan-based public or private non-profit entities with experience in rural health delivery or workforce development, including but not limited to community health organizations, local health departments, healthcare providers, hospitals, rural health clinics, and consortia that serve rural areas of Michigan.
