Synopsis As part of RHTP’s Interoperability in Action Initiative, the Rural Data Integration and Infrastructure Fund aims to the advance technology innovation through reducing duplicative care improving health outcomes through interoperable systems. This funding opportunity will advance interoperability and digital infrastructure for rural and left-behind providers to strengthen care coordination and reduce administrative burden.
Eligible applicants are rural skilled nursing facilities operating within the State of Michigan. Facilities must demonstrate capacity to integrate interoperable health data systems to enhance care coordination and reduce administrative burden. Additional eligibility criteria may be outlined within the program’s application instructions.
