Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Funds aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and well being of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships.
Eligible applicants include Michigan-based nonprofit organizations, local health departments, rural health clinics, critical access hospitals, federally-qualified health centers, tribal health organizations, and consortia serving rural populations as defined by the Michigan Department of Health and Human Services.
