Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Funds support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and well-being of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships.
Eligibility is limited to rural health organizations, health care providers, and community-based entities serving rural communities within the State of Michigan. Eligible applicants must demonstrate capacity to form cross-sector partnerships to expand access to care and improve health outcomes.
