Summary

Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Funds support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and well-being of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships.

Eligibility

Eligible applicants include rural health networks, nonprofit organizations, county health departments, federally qualified health centers, and hospital-based clinics serving rural Michigan communities, as detailed in the RHPCD-2027 Application Form.

Requirements Checklist

• Applicants must submit a complete application package via the EGrAMS portal, including the Application Form and required supporting documents: organizational budget, project narrative, partnership documentation, and cost allocation plan. Additional compliance with the Standard Agreement Language and Indirect Costs and Cost Allocation Plan Instructions is required.
