Michigan Rural Health Integration ~ Sustainability FRCs - 2027

Summary

Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Fund aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and well-being of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships. FRCs will establish formal referral practices and strengthen coordination with health partners while increasing awareness of their capacity as health navigation resources, enhancing connections to the physical and mental health sector in support of existing education, family wellness, and basic needs navigation services.

Eligibility

Eligible applicants are nonprofit organizations, local public health agencies, Federally Qualified Health Centers, Family Resource Centers (FRCs), and rural health providers operating within Michigan rural counties, as defined by state and federal rural classification guidelines. Applicants must demonstrate capacity for cross-sector partnerships and health navigation services.

Requirements Checklist

• Applicants must submit a completed application form, budget documents, signed Standard Agreement Language, and a Cost Allocation Plan. Applications must describe strategies for rural health integration, sustainability, formal referral and coordination practices, and approaches for expanding behavioral health and prenatal care access. Indirect cost rates and allocation methods must comply with provided instructions.
