Michigan Rural Health Collaborative Care Mobile Health - 2027

Summary

Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Funds aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and well being of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships.

Eligibility

Eligible applicants include Michigan-based rural health clinics, federally qualified health centers, hospitals, local health departments, local public health agencies, non-profit organizations with demonstrated experience in rural health service delivery, and tribal health organizations. Applicants must demonstrate the capacity to serve rural communities and form collaborative partnerships for the delivery of integrated mobile health and behavioral health services.

Requirements Checklist

• Applicants must submit a complete application through the EGrAMS system including: project narrative, budget and justification, signed assurances and certifications, work plan, letters of collaboration (if applicable), and proof of nonprofit or eligible entity status. All required attachments must be uploaded by the deadline. Proposed projects must address requirements for integrated, community-driven, and sustainable rural health initiatives related to mobile health delivery.
