Michigan Diabetes and Hypertension Management and Education - 2027

Summary

the Rural Health Transformation Program’s Chronic Disease Collaborative Care Fund, supports collaborative, community-based projects to improve diabetes and hypertension management and education in eligible rural and partially rural Michigan communities. The program funds evidence-based interventions to enhance care coordination, build sustainable rural health partnerships, strengthen the chronic disease workforce, promote data and information sharing, and increase access to chronic disease prevention and management services. Awardees must demonstrate organizational capacity, partnerships among local health providers, and a clear plan for addressing health disparities in diabetes and hypertension outcomes. MDHHS will provide up to $500,000 for selected projects with an implementation period from October 1, 2026, to September 30, 2027.

Eligibility

Applicants must be public or nonprofit organizations (including Federally Qualified Health Centers, Rural Health Clinics, and local public health departments) physically located in or primarily serving populations in Michigan rural and partially rural counties as designated in the FY2027 Rural and Partially Rural County Chart.

Requirements Checklist

• Applicants must submit a complete application via EGrAMS by the posted deadline, including proposals for evidence-based diabetes and hypertension interventions, documentation of active partnerships, a detailed budget and narrative, organizational information, and a rural service area map. Applicants must show capacity for population health management, health education, and secure health data sharing.
