The GO-NORTH School-Based Oral Health Programs (SBOHP) RFA, administered by the Foundation for Healthy Communities, provides competitive grants to New Hampshire-based organizations for three core goals: (1) Stabilize and strengthen existing rural school-based oral health programs (Goal 1, awards $50,000–$80,000 per application); (2) Launch new rural school-based oral health programs to expand access in underserved rural schools or districts (Goal 2, awards $350,000–$500,000 per application, up to 3 awards in total, with a focus on site identification, staffing, equipment, outreach, and sustainability); (3) Purchase and implement electronic dental record (EDR) systems for school-based oral health programs (Goal 3, approx. $20,000 per application, up to 10 awards; may be applied for standalone or with Goals 1/2, technical assistance provided). Eligible uses and requirements for each goal are specifically detailed in the solicitation. All activities and costs must comply with 2 CFR Part 200, and certain categories (e.g., equipment over $10,000) have special pre-approval requirements. Applicants must submit a narrative linking their proposal to program criteria, a work plan (using FHC's template), a project budget (using the template), letters of support from partners, and appropriate organizational documentation. Awards may be partial or full, and FHC may require budget or scope adjustments prior to contracting. The initial term runs through September 30, 2027, with possible non-competitive renewals through September 30, 2031, subject to performance and funding. The application process and evaluation criteria are designed to advance rural health transformation priorities through sustainable, evidence-based, and measurable approaches.
Eligible applicants are New Hampshire-based nonprofit, for-profit, and community-based organizations in good standing with the NH Secretary of State. Programs must serve a rural area as defined by the HRSA Rural Health Grants Eligibility Analyzer. Goal 1 is for organizations with existing school-based oral health programs; Goal 2 is for those launching new programs in rural schools/districts (including expanding into new schools; organizations looking to improve current locations are not eligible for Goal 2). Goal 3 (EDR implementation) is open to both existing and new programs as a standalone or combined application. Ineligible applicants include individuals and organizations with current legal proceedings involving the State of New Hampshire or any U.S. governmental entity. Partnerships between eligible organizations are encouraged but require a single lead applicant.
