Eligibility
Qualified vendors capable of providing quality improvement training and technical assistance to New Hampshire's Critical Access Hospitals (CAHs), with demonstrated statewide capacity and the ability to comply with all state and federal requirements. Vendors must be able to deliver evidence-based virtual training, facilitate HCAHPS measure improvement, collaborate with hospital and state Quality Improvement leadership, and provide all required reporting and documentation.

Requirements Checklist
Design, facilitate, and deliver patient-experience HCAHPS improvement training available to all 13 NH CAHs
Facilitate quarterly virtual learning collaboratives focused solely on HCAHPS performance improvement and best practices
Develop and deliver four live, evidence-based, 60-minute webinars per year via Microsoft Teams, covering all required HCAHPS topics
Provide individualized technical assistance and coaching, including virtual check-ins, collaboration on improvement strategies, and resource/tool development
Coordinate project kickoff and monthly/annual progress reviews with the Rural Health Quality Improvement Manager
Develop and implement Department-approved evaluation forms and annual assessments for webinar and collaborative effectiveness, including process-change analysis
Develop annual work plans and progress documentation detailing all training, topics, presenters, and process interventions, finalized in coordination with the Department
Provide written quarterly reports and detailed annual evaluations, including budget status reports, summary of deliverables, and participant data (no patient identifiers)
Maintain compliance with all State and DHHS requirements for accessibility, reporting, recordkeeping, training on civil rights, and document retention (per scope and Appendices)
