The program seeks to fund provider organizations, behavioral health and SUD treatment providers, Federally Qualified Health Centers (FQHCs), hospitals, community-based organizations, and cross-sector partnerships with capacity to deliver SUD-focused interventions that align with future value-based care (VBC) models. Funded activities must represent net new or expanded efforts beyond existing reimbursable services and demonstrate measurable, near-term improvements in SUD health outcomes, access to care, care coordination, and readiness for VBC. Required strategies prioritize upstream, root-cause-focused solutions over episodic, downstream care and include explicit integration with the State’s RHTP data spine, use of non-duplicative and non-billable activities, robust operational and outcome metrics tracking, and ongoing participation in State-led VBC design workgroups. Funds are awarded and released in milestone-based tranches: (1) program design/operational readiness, (2) initial implementation/service delivery, and (3) achievement of performance thresholds and sustainability. The goal is to expand rural/underserved service capacity, reduce avoidable emergency/crisis-related system utilization, improve population-level SUD metrics, facilitate workforce reintegration, and strengthen long-term system transformation consistent with State MAHA principles and the Governor’s Four Pillars. All grantee data, reports, and workflows become State assets for analytics, oversight, and continuity. This one-year funding opportunity, totaling $6,000,000, ends July 31, 2027; continued funding is contingent on State/CMS appropriations and demonstrated grantee outcomes, as defined by RHTP and CMS/HHS requirements.
