Summary

the State-led RHTP data spine or approved interim methods, adhering to HIPAA and 42 CFR Part 2 standards. Programs must demonstrate quantifiable clinical improvements (such as HbA1c reduction and blood pressure stabilization) and support monthly and quarterly reporting using State and CMS templates, granting the State real-time access to all generated programmatic assets. Funding advances will be provided for start-up, with subsequent milestone-based disbursements tied to validated achievement of key programmatic and deployment milestones, including device distribution to targeted clinicians and patients. Applicants must submit applications electronically through the WVOASIS Vendor Self-Serve (VSS) Portal and include a Detailed Line-Item Budget (DLIB) Worksheet and budget narrative as per RHTP Budget Instructions. The program targets 1–7 grantees, with preference for applicants manufacturing, commercializing, or expanding job-creating operations in West Virginia. Cost share/match is welcomed, not required. Award decisions will be based on stringent review criteria, including capacity, impact, plan feasibility, sustainability, and budget, with a minimum score of 80% required for consideration. All funds must be expended and invoiced by July 31, 2027.

Eligibility

Eligible applicants include Outpatient Remote Patient Monitoring vendors or clinical care organizations intending to implement OP RPM for West Virginians with Diabetes and/or Hypertension. Preference is given to applicants manufacturing, commercializing, creating jobs, establishing, or expanding operations within WV. All applicants must register and submit applications via the WVOASIS Vendor Self-Serve (VSS Portal). Multiple grantees (anticipated 1–7) may be selected, with priority for high-need rural county service.

Applicants must: 1) Deploy FDA-cleared OP RPM devices to high-risk populations with diabetes and/or hypertension in targeted rural counties; 2) Rapidly operationalize programs within 60 days of award; 3) Integrate clinical and device data with provider EHRs and directly to the State RHTP data spine (or approved interim method); 4) Embed nutrition and root-cause-focused lifestyle interventions directly into clinical workflows, leveraging local partnerships; 5) Onboard and monitor a defined cohort of eligible, high-risk participants with uncontrolled A1Cs (>9) and/or hypertension (>130/80); 6) Collaborate across RHTP initiatives for referrals and community-based workforce engagement; 7) Track and report measurable patient outcomes (e.g., HbA1c reduction, BP stabilization, workforce participation improvements) using standardized State/CMS methodologies; 8) Submit monthly and quarterly financial and program reports using prescribed templates; 9) Include a DLIB Worksheet and budget narrative as per RHTP Budget Instructions. Funding may not be used to supplant existing activities, or as cost share for other programs without written approval. Data, dashboards, and reports are State program assets. All funds/invoices must be expended/submitted by July 31, 2027.
