The program emphasizes outcomes-driven virtual care adoption, expanded patient access, and sustainable provider participation linked to CMS and state healthcare transformation goals. Applications must provide a detailed launch plan, line-item budget, and describe data integration and compliance processes, with adherence to strict maintenance of effort and anti-supplanting requirements. All reports and deliverables must be accessible to the State for oversight and integration, and awardees must comply with State and federal privacy, data, and audit requirements.

Requirements Checklist

• Applications must be submitted electronically via the WVOASIS Vendor Self Service (VSS) Portal and by email to the designated program contact. Required attachments include: 1) Detailed Line-Item Budget (DLIB) worksheet and budget narrative, 2) RHTP Grantee Contact Information Form, 3) Detailed implementation and milestone plan showing operational and technical readiness for telehealth upgrades, 4) Proof or plan for integration with the WV Health Information Network (WVHIN), 5) Evidence of applicant eligibility (e.g., nonprofit or West Virginia-based status), and 6) Other documentation as required by AFA Section Six. All cost and documentation submissions must comply with State and CMS reporting requirements and may be subject to financial and programmatic audit. Award recipients must provide monthly and quarterly programmatic and financial reports and meet all federal and state compliance standards, including HIPAA and 42 CFR Part 2 protections as applicable. Awarded funds must be used to supplement (not supplant) existing activities or staffing. Subrecipients must participate in State-led governance activities and provide timely, standardized participant-level and aggregate outcome data as specified by the State.
