The State of West Virginia has released an Announcement of Funding Availability (AFA) for Year 1 grants under its Rural Health Transformation Program (RHTP), specifically for the Connected Care Grid (CCG) Inpatient Remote Patient Monitoring (RPM) initiative. Up to $4,915,066 is available in total for 3–4 grantees to implement inpatient-anchored RPM models leveraging connected monitoring technologies and virtual clinical workflows. These models aim to improve care delivery for high-risk and rural populations, reduce hospital length of stay, accelerate care transitions, reduce unnecessary readmissions and emergency department utilization, and generate actionable data to inform statewide performance monitoring. Grants are performance-based, reimbursed by milestones: start-up, 50% bed deployment, and full bed deployment with health outcome improvement. All core capabilities must be live, integrated, and actively used by the end of Year 1—this is not a planning or pilot effort. Activities executed under this AFA must not duplicate or supplant existing value-based or mandated programs, nor replace current funding or staffing. Required operational metrics include device deployment and utilization, patient adherence, provider satisfaction, and reductions in length of stay and ED visits. Eligible applicants include acute care hospitals, critical access hospitals, hospital-based behavioral health facilities, skilled nursing operators, or other entities approved by the State. Funding must be expended and invoiced by July 31, 2027. Proposed activities must demonstrate supplemental enhancement consistent with RHTP goals, CMS requirements, and the State's evaluation framework. Applicants must submit monthly and quarterly quantitative and qualitative reports. CMS and State reporting, governance participation, and real-time State access to all generated data are required.
Eligible applicants are acute care hospitals, critical access hospitals, hospital-based behavioral health facilities, skilled nursing operators, and other entities approved by the State that can implement inpatient-based RPM programming consistent with RHTP requirements. Applicants must demonstrate that funded activities supplement or expand current services and do not replace existing funding or staffing. Preference is given to rural and underserved communities statewide.
