ToRCH Care Smart Growth and Service Line Modification (Horizon 2) Invitation For Bid

Eligibility

Eligible applicants: Rural hospitals and health centers licensed under Missouri law—including but not limited to general hospitals, Critical Access Hospitals, Rural Emergency Hospitals, safety-net hospitals, Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), Certified Community Behavioral Health Clinics (CCBHCs), Behavioral Health Clinics (BHCs), Certified Community Mental Health Centers (CMHCs), birthing centers, urgent care centers, primary care centers, and hospital- or health center-affiliated entities applying on behalf of an eligible rural hospital or health center. Projects must serve an RHTP-designated rural county, and public, nonprofit, and qualifying private entities are eligible. Requirements Checklist

• Submit a single comprehensive application per eligible entity unless otherwise approved. 2. Project must directly align with one or more Horizon 2 priority service lines: maternal health, behavioral health, or diagnostics/outpatient/chronic disease management. 3. Projects must be for service modernization, renovation, reconfiguration, or equipment of existing facilities (no new construction or major expansion). 4. Projects must serve RHTP-designated rural counties. 5. Maximum request: $5,000,000 per proposal. 6. At least 25% cost share (match) is required unless a hardship waiver is granted. 7. Applicants must complete and submit all required IFB exhibits and certifications: Technical Methodology/Work Plan (Exhibit A/A-1/A-2), Past Performance (Exhibit B), Hospital Information (Exhibit C), Budget Narrative (Exhibit D), and Exhibits 1–5 per IFB instructions. 8. Projects must be implementation-ready within one month post-award and completed by September 30, 2028 (Phase 2 funding beyond 2027 contingent on CMS approval). 9. Funds must not supplant, duplicate, or replace other public or private funding sources; ineligible costs include new construction, food or routine admin/operating costs, or non-healthcare equipment. 10. Preference for applications with regional or multi-county reach, strong partnerships, documented access gap, and clear sustainability plan. 11.
