the Rural Health Transformation Program (RHTP) funded by CMS/HHS. This IFB funds rural hospitals and health centers to modernize, renovate, reconfigure, or equip existing rural healthcare facilities in alignment with Horizon 2 priorities: (1) maternal health infrastructure (obstetric, perinatal, prenatal, postpartum, and women's health services); (2) behavioral health infrastructure (inpatient, outpatient, crisis, tele-behavioral health, integrated behavioral health); and (3) diagnostics/outpatient/chronic disease management infrastructure (imaging, laboratory, specialty consults, care coordination, and longitudinal management). Projects must directly serve RHTP-designated rural counties, address documented local service-line gaps, sustain expanded access beyond the grant period, and demonstrate readiness for implementation. All activities are limited to modernization and reconfiguration of existing facilities (no new construction or major expansions), must comply with CMS/HHS guidance and state law, complement other rural transformation efforts, and support transition toward alternative payment models. Funding is 100% federal (CMS/HHS); a 25% cost-share is required, with waiver possible in cases of financial hardship. Maximum award per proposal is $5,000,000; total program funding is anticipated at $40 million for the round, subject to final CMS budget approval. Eligible applicants: Rural hospitals and health centers licensed under Missouri law—including but not limited to general hospitals, Critical Access Hospitals, Rural Emergency Hospitals, safety-net hospitals, Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), Certified Community Behavioral Health Clinics (CCBHCs), Behavioral Health Clinics (BHCs), Certified Community Mental Health Centers (CMHCs), birthing centers, urgent care centers, primary care centers, and hospital- or health center-affiliated entities applying on behalf of an eligible rural hospital or health center. Projects must serve an RHTP-designated rural county, and public, nonprofit, and qualifying private entities are eligible.
