This opportunity invites eligible lead entities to apply for up to $59.2 million in Year 1 federal funding as part of Utah's Rural Health Transformation Program (RHTP) under the SPRINT Consortium. The grant supports rural health infrastructure, workforce development, technology modernization, and care innovation through a consortium model. Eligible applicants must demonstrate operational status, fiscal capacity, geographic diversity, and engage rural hospitals, clinics, FQHCs, and independent rural providers as stakeholders. Applications must propose activities aligning with strategic RHTP goals and comply with specific category limits. The agreement period runs from September 11, 2026, to September 30, 2031, and future funding is outcome-dependent. Only one lead entity is the primary recipient, with partners as subrecipients. The SPRINT Consortium aims to drive strategic advances in infrastructure, sustainable provider revenue, and network technology for rural Utah.
Eligible lead entity applicants must be legal entities registered in Utah (for-profit, non-profit 501(c)(3), or government/public) and not debarred, suspended, or otherwise excluded from federal or state funding. They must attest to business and fiscal capacity, have an active Utah vendor registration, and submit a letter of intent by the deadline. The applicant must represent a consortium including rural hospitals, rural clinics, FQHCs, and independent rural providers. Geographic diversity and formal documentation of stakeholder governance and active roles are required. All consortium members must provide letters of collaboration.
