Requirements Checklist

• Required submissions include a detailed project narrative (with sections on statement of need, project description and program alignment, partnerships, baseline data, sustainability plan), a work plan and timeline (template required), a budget and justification (template required), a performance metrics and evaluation plan (template required), an organizational chart (template required), and combined resumes/CVs of key project personnel. Conditional required documents include audited financial statements, IRS determination letters, Board of Directors lists, and letters of support or MOUs from key partners, as applicable. All files must be uploaded electronically, with a file size maximum of 50MB per item.
