The Rural Health Transformation Program (RHTP) - SHIFT Initiative (Sustaining Health Infrastructure for Transformation), administered by the Utah Department of Public Safety, Bureau of Emergency Medical Services (BEMS) with support from Utah Department of Health and Human Services (DHHS) under a CMS-funded award, will provide $12,500,000 in Year 1 with an estimated $10,000,000 annually for Years 2-5 to sustain and strengthen rural and Tribal EMS infrastructure in Utah. The competitive, multi-year program funds projects that expand EMS access, modernize health communications, and build sustainable EMS capacity by supporting education and workforce development, community paramedicine and telehealth, innovation and technology, and operational/administrative support. Eligible projects must align with at least one of four program areas: Education (including workforce development and training), Community Paramedicine and Telehealth, Innovation and Technology, or Administrative Support. The SHIFT Initiative prioritizes solutions that improve rural EMS service, enhance communication, and foster long-term, sustainable improvements in rural and Tribal communities. Applications are evaluated primarily for Years 1–2. Subgrants are annual, with project periods defined per funding year, and strict budget and use restrictions apply.
Eligible applicants are hospitals or health systems, Federally Qualified Health Centers (FQHCs) or Rural Health Clinics (RHCs), Emergency Medical Services (EMS) agencies, Tribal health organizations, and educational institutions serving rural Utah counties or Tribal Nations (including, but not limited to, the Navajo Nation, Ute Indian Tribe, Paiute Indian Tribe, Goshute Tribes, and Shoshone Nation). Applicants must serve rural Utah counties or federally recognized Tribal Nations within Utah.
