The program aims to strengthen rural health system capacity and sustainability, with up to $10M available in Year 1 and $45M total over five years. Up to 12 awards will be made, with up to 5 reserved for tribes or tribal organizations that meet full RFGA criteria.
Eligibility
Legal entities in Utah or border states serving Utah residents. For-profit, non-profit, government/public, and tribal organizations. Must meet registration, standing, and federal/state funding eligibility.
Applicants must submit one complete application per entity through the official Qualtrics portal provided by Utah DHHS. All required attachments (see 'Required Attachments') must be included, using the official DHHS budget template. Entities must attest to eligibility and registration intent as specified in the RFGA and furnish all documentation supporting legal eligibility. Project narratives must address operational readiness, required service models, sustainability strategies, and compliance with all funding restrictions. Incomplete or late applications will not be considered. All proposed service activities must occur in eligible rural Utah counties by entities not headquartered in excluded counties.
