This opportunity offers up to $2.5 million in federal funding for Utah-based multi-facility rural health provider networks to expand services, strengthen operational and clinical integration, improve emergency medical services, and promote financial sustainability. Applications are sought from partnerships including at least three different rural facility types, such as CAH, REH, RHC, I/T/U, or FQHC facilities. Up to six awards may be made for Budget Period 1 (September 21, 2026, to September 30, 2031). Bonus scoring is available for projects strengthening tribal health infrastructure. Key deliverables and expectations include tracking implementation metrics, continuous progress reporting, and developing a sustainability plan for the rural health network beyond the grant period.
Eligible applicants must be a Utah-based non-profit 501(c), for-profit entity, government/public entity, or a formal health network (established or emerging). Applications must include at least three different types of participating rural facilities (for example: CAH, REH, RHC, I/T/U, FQHC). Entities must provide at least two pieces of validation evidence of collaboration (such as MOUs or joint initiatives). Applicants may not be debarred, suspended, or excluded from receiving federal or state funding.
