This opportunity supports the Smart Care Catalyst initiative under the West Virginia Rural Health Transformation Program (RHTP), funded via CMS/HHS as part of an approximately $200 million federal grant. The SCC2 VBC Design & Administer grant, RHT-AFA-07-27-2026-SCC2, solicits applications from qualified entities to design and administer standardized, multi-payer value-based payment models targeting substance use disorder (SUD) in rural West Virginia, beginning with initial implementation in the southern counties and structured for statewide scaling. The awarded entity will: (1) define standardized, multi-payer VBC model structures and deliver implementation-ready model packages; (2) establish performance measurement and data-sharing frameworks (with integration to the State RHTP data spine and WVHIN); and (3) convene, align, and develop governance with Medicaid MCOs, other payers, and providers. Funding will be tied to milestone and deliverables approved by the State—focused on health outcomes, reduced avoidable utilization, and improved workforce participation. All work products must be accessible to the State as assets; data reporting must conform to CMS and RHTP requirements; and non-performance will trigger reallocation of funds. Awards are milestone-based and for Year 1 only (through July 31, 2027); future funding is contingent on State receipt of federal continuation funds and satisfactory grantee performance.
Eligible applicants must demonstrate experience in value-based payment model design, administration, and multi-payer coordination. Eligible organizations include, but are not limited to: value-based care enablement organizations; population health management entities; accountable care organizations (ACOs); clinically integrated networks (CINs) or similar provider-led collaboratives; health system-affiliated or independent management services organizations (MSOs) or shared services entities; academic or research institutions with payment model expertise; and other organizations with operational, analytic, and governance capabilities to design and administer statewide, multi-payer value-based payment models. Applicants must show ability to coordinate with payers/providers, align with CMS requirements, and support RHTP data integration, including compliance with applicable privacy and data security (HIPAA/42 CFR Part 2) standards.
