The State of Arizona is soliciting stakeholder input regarding the development of Arizona’s application to the Rural Health Transformation Program (RHTP), which seeks federal CMS funding to improve healthcare access and quality of care in rural Arizona. This Request for Information (RFI) is not a grant application or RFP, but invites feedback on possible uses for RHTP funding, with an emphasis on addressing healthcare challenges exacerbated by federal policy changes (H.R. 1) and anticipated increases in uncompensated care. Input will inform Arizona's RHTP proposal to CMS.
Any stakeholder or party with input on rural health needs or potential uses of RHTP funds in Arizona.

Program: Rural Health Transformation Program (RHTP) Review Note: Genuinely RHTP-specific: AZ stakeholder-input RFI ahead of its state RHTP application, per Arizona Revised Statutes sec 41-2555. Past due (Sept 2025) but kept as a historical record of AZ RHTP planning -- program has since fully matured (State Plan + multiple approved RFAs/Grants). No duplicate among AZ approved records.
