AHCCCS issues this sixth public solicitation under the Arizona Rural Health Transformation Program (RHTP): Request for Grant Applications (RFGA) YH-27-0025, to support Adopt Shared Services Consortiums. This RFGA funds development and implementation of shared service models among rural and Tribal healthcare providers across rural Arizona. Projects may feature shared staffing models (joint employment, traveling clinicians, locum tenens), shared training/education programs, joint IT/data infrastructure, centralized billing/coding, common scheduling, co-located service hubs, supply chain cooperatives, shared compliance functions, and regional service networks, aiming to reduce operational costs, eliminate duplication, and enhance financial and clinical capacity. Three funding tiers support projects from two organizations up to regional hub models. Strict administrative and capital expenditure caps apply. Funded projects must create new or expanded shared-service activities, not supplant or duplicate existing service or funding streams. Tribal partnerships are encouraged and prioritized.
Eligibility
Eligible applicants are limited to small rural clinics, Tribal health programs and 638 facilities, community-based organizations (including providers) serving rural populations, rural EMS agencies and fire departments, school-based health centers in rural areas, Rural Health Clinics (RHCs), Federally Qualified Health Centers (FQHCs), small Critical Access Hospitals, county health departments, and rural provider consortiums. Tribal facilities and inter-Tribal consortiums may apply to any tier for which they qualify. Eligibility extends only to entities serving HRSA-designated rural service areas in Arizona (see https://data.hrsa.gov/topics/rural-health/rural-health-eligibility for HRSA definition). Applicants must comply with the RFGA’s full eligibility criteria.

Requirements Checklist
Projects must establish new or expanded shared-service activities such as shared staffing, training, billing/coding, IT/data systems, scheduling, facilities, joint procurement, or regional hub models. Tier 1 supports small-scale, single-function arrangements (2–3 orgs), Tier 2 supports multi-function projects with formal governance (4–6 orgs), Tier 3 supports comprehensive regional hubs (7+ orgs/networks). Activities may not duplicate, sustain, replace, or supplant existing funding/services or services already reimbursed by Medicaid/Medicare/other third-party payers. Administrative costs by grantee are capped at 7% per award; additional direct and indirect cost restrictions apply to capital/infrastructure (20%), provider payments (15%), and EMR/EHR replacements (5%). Projects must be implemented in HRSA-designated rural areas. All costs must be obligated within the budget period and liquidated by federal deadlines. Compliance with budget-period obligation, reporting, and Stevens Amendment public communication rules required.
