This competitive grant opportunity, funded under the CMS Rural Health Transformation Program (RHTP) and administered by AHCCCS, is designed to foster shared service models that increase operational efficiency and sustainability among rural and Tribal health providers in Arizona. Three funding tiers are available, supporting projects from single-function arrangements (e.g., shared billing) to comprehensive regional shared-services hubs. Eligible activities include joint staffing, collaborative training, shared data systems, facilities co-location, joint purchasing, and compliance collaboration. Emphasis is placed on forming self-sustaining rural provider networks and long-term shared service hubs. Funding draws from Arizona’s FY26 RHTP allocation of $166,988,956, subject to a 10% administrative cap (7% for subrecipients), and all proposed activities must represent an expansion or enhancement of current services, not a replacement. Applicants must demonstrate measurable improvements in efficiency, capacity, and scope for rural and Tribal populations, with bonus evaluation points for Tribal engagement and projects serving fully rural counties.
Eligible applicants are limited to small rural clinics, Tribal health programs and 638 facilities, community-based organizations (including providers) serving rural populations, rural EMS agencies and fire departments, school-based health centers in rural areas, Rural Health Clinics (RHC), Federally Qualified Health Centers (FQHCs), rural health networks, and health-related nonprofit organizations, as designated by AHCCCS in accordance with RHTP guidance.
