This RFGA is the seventh public solicitation by AHCCCS under the Arizona Rural Health Transformation Program (RHTP). AHCCCS issues this RFGA to consolidate three program areas: (1) expansion of mobile and digital behavioral health services and crisis stabilization infrastructure; (2) training, recruitment, and retention of behavioral health professionals in rural shortage areas; and (3) community prevention programming addressing suicide, trauma, and substance use, specifically targeted to rural and rural Tribal communities in Arizona. The RFGA provides up to $8.5 million in combined funding per budget year across these areas, employing a tiered approach to match organizational capacity and proposed scope. Projects must represent new or expanded activities beyond current operations and are expected to increase access to behavioral health services, strengthen regional workforce pipelines, and improve population health outcomes. Eligible entities include a wide range of rural health and behavioral health agencies, organizations, clinics, health systems, schools, Tribal programs, and coordinated coalitions as detailed within each program area and funding tier.
Eligibility
Eligible applicants include: behavioral health managed care plans, Tribal behavioral health authorities, rural public health agencies, rural community-based organizations, rural health clinics, behavioral health providers, Critical Access Hospitals, Federally Qualified Health Centers (FQHCs) serving rural areas, Tribal health facilities, rural health systems, anchor institutions (hospitals, universities, nonprofit healthcare systems), Tribal consortiums, multi-stakeholder collaborative networks, and ISA/IGA-eligible entities. Prevention program applicants may include rural school districts, Tribal education and health programs, county public health agencies, youth-serving community-based organizations (CBOs), and other health providers serving rural or Tribal populations in Arizona. Entities must demonstrate proposed work directly benefits rural and/or rural Tribal Arizona populations.

Requirements Checklist
Applicants must propose projects that represent new or expanded activities outside existing operations. Awardees may request funding under one or more program areas (Mobile/Digital Services, Training & Recruitment, Prevention Programs) but may apply for only one tier per program area per application. Funding is subject to annual CMS approval and satisfactory performance. Administrative costs (direct+indirect) are capped at 7% of the total subaward. Provider payment-related expenditures are capped at 15%, infrastructure/capital expenditures at 20%, and EMR/EHR replacement at 5% (where applicable). All activities must comply with 2 C.F.R. Part 200 and federal subrecipient monitoring and reporting requirements. Funds must support new/expanded effort and may not supplant existing funding. All award funds must be obligated within the specified budget period; no carryover or no-cost extensions.
