REH Conversion/Transformative Capital Investment Grant Program (REH/CAP)

Requirements Checklist

• Projects must align with at least one program objective, demonstrate measurable transformation, and comply with allowable use of funds (minor renovations, equipment, vehicles, technology, etc.; no new construction or major expansion). Indirect/admin costs limited to 10% of the award. Salaries must comply with federal cap ($225,700). Provider payments and broadband infrastructure are not allowed. Reporting includes quarterly and annual evaluation submissions.

• Completed and signed W-9 form
• Evidence of active UEI registration
• Budget narrative (max 3 pages)
• Project description
• Implementation plan
• Sustainability plan
• Evaluation plan

Submission Instructions

Applications must be submitted via the KDHE online application portal by 12:00 PM CT on 03/20/2026. Required attachments must be uploaded in PDF/Excel format.
