Community Health Worker + Accountable Food Is Medicine Grant Program (CHW+AFIM) – Year 2

Summary

Year 2 continuation of the Community Health Worker (CHW) + Accountable Food Is Medicine (AFIM) grant program under Kansas's Rural Health Transformation Plan. KDHE invites eligible rural Kansas healthcare organizations to apply to integrate CHWs into clinical settings delivering AFIM interventions. CHW+AFIM aims to improve health outcomes for rural Kansans by integrating CHWs and AFIM interventions targeting chronic disease. Year 2 features expanded evaluation support, required reporting, sustainability planning, and development of public/private payment integration for CHWs. Up to seven proposals will be funded; each award ranges from $100,000 to $210,000. Applications due 11/18/2026 12PM CT; awards announced 12/18/2026; performance period 12/1/2026-9/30/2028.

Eligibility

Any rural, Kansas-based healthcare organization may apply, including federally qualified health centers, rural health clinics, hospitals, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, FQHC look-alike clinics, and primary care clinics. Applicants must possess an active Unique Entity Identifier (UEI), Federal Employer Identification Number (FEIN), and SMART Vendor ID.
