the state's Rural Health Transformation Plan and funded by the federal Rural Health Transformation Program (RHTP). The program aims to support investments that transform care delivery, reduce chronic disease and avoidable hospitalizations, improve provider-to-population ratios for primary care, dental, and behavioral health services, and enable telehealth innovation and data sharing. Applicants must propose projects that align with at least one program objective and demonstrate measurable outcomes. Typical projects include equipment, vehicles, software, minor renovations, and infrastructure upgrades. Funding cannot be used for new construction, broadband infrastructure, provider payments, lobbying, or other specifically disallowed activities by CMS.
Eligible providers are Kansas hospitals, physician practices (including solo practitioners), rural health clinics, provider networks and consortia, federally qualified health centers, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, licensed substance use disorder facilities, and licensed long-term care facilities located in rural areas as defined by HRSA. All applicants must have an active UEI, FEIN, and SMART Vendor ID.
