the state's Rural Health Transformation Plan. Funded projects must align with at least one RPGP objective: reducing chronic disease and avoidable hospitalizations; strengthening rural provider financial viability; expanding workforce capacity; increasing the proportion of Medicare and Medicaid beneficiaries in accountable care relationships; or enabling rural providers to engage in data sharing, telehealth, appropriate AI, and consumer-facing technology use. Administrative and indirect costs are capped at 5%. Anchor hospitals are excluded from eligibility. Awards will prioritize statewide impact and measurable results toward program objectives.
Any Kansas-based organization that has secured a commitment from at least one Eligible Provider (hospital, physician practice, rural health clinic, provider network/consortium, FQHC, local health department, tribal health facility, community behavioral health clinic, licensed SUD facility, or licensed long-term care facility located in a rural area) may apply. Providers in non-rural areas may apply if the collaborative arrangement directly benefits rural communities. Anchor hospitals designated under Kansas RHTP Program 2.4 are not eligible. Applicants must possess a UEI, FEIN, and SMART Vendor ID.
