North Dakota Health and Human Services (ND HHS), through the Rural Health Transformation Program (RHTP), offers the Technology as an Extender funding opportunity to support North Dakota rural and tribal health facilities in implementing smart technology, robotics, and AI-based solutions that address provider shortages and advance care delivery for rural residents. This competitive opportunity allocates approximately $5 million in federal funds for up to 20 awards, each averaging $250,000. Funded projects may include acquisition and staff vendor training for remote patient monitoring (facility-based only), AI-enabled digital engagement, care coordination tools, robotics for clinical and environmental efficiency, and electronic caregiver systems. Priority is given to projects introducing equipment not currently owned or in active use by the applicant, particularly those that improve organizational efficiency, enhance safety and care quality, reduce the rural cost burden, and measurably impact Initiative 2 metrics: rural provider capacity, care quality, and access. All equipment and technology purchases must align with eligible uses. Awardees are required to report on workforce and project impact metrics for up to five years, in accordance with Initiative 2 of the state's RHTP, and are subject to quarterly and annual reporting. Award funds cannot supplant existing funding, duplicate other program awards, or support unallowable costs as defined by state and federal guidelines.
Eligible applicants are North Dakota rural healthcare facilities serving populations that are at least 50% rural residents or where funded activity is exclusively for rural residents. Eligible facility types include: Critical Access Hospitals (CAHs), Rural Emergency Hospitals, non-federally operated Indian Health Services facilities and tribally run 638 facilities, clinics, Federally Qualified Health Centers (FQHCs) serving at least 50% rural North Dakota citizens, Skilled Nursing Facilities (SNFs), and Basic Care Facilities. Entities or project sites within Grand Forks, Fargo, West Fargo, and Bismarck city limits are only eligible if at least 50% of the service population is rural or the project exclusively benefits rural residents. RHTP funds must exclusively support rural North Dakota citizens.
