Summary

the Rural Health Transformation Program (RHTP) Initiative 3: Bring High-Quality Healthcare Closer to Home. This Year 1 award, 'Coordinating and Connecting Care' (Solicitation 210-362), will issue approximately $1,000,000 in federal funds to support planning for a neutral, statewide Community Information Exchange (CIE) and closed-loop referral system focused on rural and Medicaid-serving providers. The grant aims to strengthen cross-sector partnerships, assess care coordination gaps, and lay the groundwork for state-wide decision-making on future care coordination, referral, and secure information exchange infrastructure for rural and tribal populations. Year 1 funding supports planning only and does not authorize implementation or procurement of a statewide system. Eligible applicants include HIE/CIE organizations, rural health networks, healthcare associations, collaboratives, higher education institutions, research organizations, community-based or volunteer organizations, payer/managed-care organizations, and governmental or quasi-governmental entities. Applicants must demonstrate a rural focus, but do not need to be rural-located. Partnerships are allowed with a designated lead applicant/fiscal agent. All applications must be submitted via Qualtrics no later than August 26, 2026, 5:00 p.m. CT.

Eligibility

Eligible applicants are Health Information Exchange (HIE)/Clinical Information Exchange (CIE) organizations, rural health networks, associations or collaboratives, higher education or research organizations, community-based or volunteer organizations, payer/managed-care organizations, and governmental or quasi-governmental entities. Applicants do not have to be located in rural areas but must demonstrate a clear rural focus in their proposed project. Partnerships and consortiums may apply but must designate one eligible entity as the lead applicant and fiscal agent.

Requirements Checklist

Applicants must submit a completed Qualtrics application, including responses to all sections and an uploaded 'Itemized Subrecipient Budget Template' in Excel format. Required content includes organizational background, demonstration of eligibility, proposed project narrative with detailed outcomes, strategies, and tasks, description of stakeholder engagement plans, sustainability recommendations, and metrics for measuring planning progress. Only one application per organization may be submitted. All funded activities must be completed by August 31, 2027. Applications must not include confidential/proprietary information, as submissions become open records. Reporting on milestones and quarterly/final outcomes is required.
