North Dakota Health and Human Services (ND HHS), through the Rural Health Transformation Program (RHTP), offers competitive funding to support enhancements to existing Electronic Medical Record (EMR) systems among eligible rural and tribal health providers. This funding opportunity, as outlined under Initiative 4: 'Connect Tech, Data and Providers for a Stronger North Dakota', Theme 1: 'Breaking Data Barriers—Enhance existing EHRs with additional technology and AI add-ons,' is focused on proposals that directly strengthen service delivery, improve patient outcomes and experience, increase access to care, reduce wait times, and reinforce cybersecurity. Only projects enhancing established EMR systems are eligible—EMR replacements are addressed in separate, future opportunities. Up to $10 million is available in Year 1, with estimated average awards of $500,000 and approximately 20 anticipated recipients. Projects must use the ND HHS Itemized Subrecipient Budget Template (Excel; one tab per strategy) with cost justifications. Projects must be fully implemented and all funds expended by September 30, 2027. Priority is given to applications demonstrating measurable improvements in patient outcomes, expanded access, and reductions in wait times for rural/tribal residents. Awardees are ineligible for subsequent RHTP EMR replacement funding. Only one application per eligible organization is permitted.
Eligible applicants must serve rural or tribal North Dakota residents. Eligible rural organizations include: critical access hospitals; rural federally qualified health centers; rural hospital systems; non-federally operated Indian Health Service facilities and tribally run 638 facilities; and long-term care facilities. Entities located in or incorporated into Grand Forks, Fargo, West Fargo, and Bismarck are urban and ineligible. Projects must benefit rural or tribal residents and be sustainable. Awardees may not seek future RHTP EMR replacement funding. Each eligible entity must apply individually. No pass-throughs or subawards are permitted.
