This is a competitive funding opportunity through North Dakota Health and Human Services' Rural Health Transformation Program (RHTP), supporting the expansion of telehealth infrastructure in rural and tribal communities lacking local healthcare providers. The goal is to create and enhance telehealth access points—such as telehealth pods in community facilities, mobile telehealth units, and technology upgrades—that enable rural Medicaid members and other underserved residents to access preventive services, behavioral health, specialty care, and chronic disease management. Approximately $3.6 million is available in this cycle, with an estimated 25 individual awards averaging $144,000 each. Funded projects are expected to strengthen rural care delivery capacity, increase access, and improve measured health outcomes. The operating period concludes August 31, 2027, and all awarded funds and equipment must be expended and received by that date.
Eligible applicants are rural healthcare organizations in North Dakota, including hospitals, clinics, home care and long-term care providers, behavioral health providers, emergency medical services, pharmacy providers, local public health units, Tribes and tribal organizations (excluding federally operated Indian Health Services facilities). Projects must serve rural residents; urban-based applicants are only eligible if at least 50% of the project serves ND rural citizens. Priority will be given to projects improving access and outcomes for rural, tribal, medically underserved, or frontier communities without local healthcare providers.
