North Dakota Health and Human Services, through the Rural Health Transformation Program (RHTP), announces competitive funding under Clinics without Walls: Remote Patient Monitoring (Solicitation 210-334). Approximately $1.6 million in federal funds will be awarded to eligible rural healthcare providers—including rural hospitals, critical access hospitals and their partner clinics, federally qualified health centers, rural clinics, rural PPS systems, and eligible tribal organizations—to implement and expand remote patient monitoring (RPM) technology. Projects must serve rural North Dakota residents, with priority for new RPM equipment and Medicaid-focused initiatives. Fundable activities include purchasing and implementing new RPM devices/applications, integrating monitoring and telehealth with electronic health records, technical/training support, and patient coaching. Awards average $80,000, with an operating period through August 31, 2027. Applications are due August 14, 2026, at 5:00 p.m. CT via Qualtrics. Only one application per organization is permitted.
Eligible applicants are rural healthcare providers serving North Dakota rural residents. This includes rural hospitals, rural emergency hospitals, critical access hospitals and their partner clinics, federally qualified health centers, rural clinics, rural PPS systems, and tribes or tribal organizations (excluding federally operated Indian Health Services facilities, per CMS requirements). Entities in Grand Forks, Fargo, West Fargo, and Bismarck are considered urban and are only eligible if at least 50% of the population served, or the primary project benefit, is for rural North Dakota citizens. Applicants must use funds to support rural North Dakota citizens, including rural tribal residents.
