Michigan Rural Community Health Workforce Recruitment Retention - 2027

Summary

The grantee will serve as the subrecipient to establish a baseline that can be used to establish targets and demonstrate CHW-related outcomes. The funds will support coordinated outreach, data collection, analysis, and reporting necessary to produce a reliable workforce inventory and needs assessment, ensuring efficient use of resources while avoiding duplication of existing efforts.

Eligible applicants include Michigan-based public health departments, tribal health organizations, and rural community health agencies. See Application Form for specific eligibility criteria.

Requirements Checklist
