As part of RHTP, the 5% Tribal Carve-out will strengthen regional partnerships to better align service delivery and promote sustainable care models. This initiative will support the development of care teams and infrastructures that integrate health professionals with community health workers to address culturally specific needs. It will also advance technology and interoperability to improve care coordination, and establish referral networks that connect residents to local care, prevention, and wellness resources that foster long-term healthy living.
Eligible applicants are Michigan federally recognized Tribal Nations and organizations serving rural Tribal populations, as specified in the funding announcement.
