Michigan Rural Health Tribal Initiative - 2027

Summary

As part of RHTP, the 5% Tribal Carve-out will strengthen regional partnerships to better align service delivery and promote sustainable care models. This initiative will support the development of care teams and infrastructures that integrate health professionals with community health workers to address culturally specific needs. It will also advance technology and interoperability to improve care coordination, and establish referral networks that connect residents to local care, prevention, and wellness resources that foster long-term healthy living.

Eligibility

Eligible applicants are Michigan federally recognized Tribal Nations and organizations serving rural Tribal populations, as specified in the funding announcement.

Requirements Checklist

• Applicants must submit a completed application form and comply with the standard agreement language. Required documentation may include an indirect cost and cost allocation plan, organizational information, project narrative, budget justification, and assurances regarding service delivery and reporting.
