Michigan Rural Health Interoperability Data Integration - 2027

Summary

Synopsis As part of RHTP’s Interoperability in Action Initiative, the Rural Data Integration and Infrastructure Fund aims to the advance technology innovation through reducing duplicative care improving health outcomes through interoperable systems. This funding opportunity will advance interoperability and digital infrastructure for rural and left-behind providers to strengthen care coordination and reduce administrative burden.

Eligibility

Eligible applicants include rural health providers, hospitals, community health centers, and health clinics operating in the state of Michigan. Additional eligibility details and requirements may be provided in the EGrAMS portal or the solicitation.

Requirements Checklist

• Applicants must submit a completed application form via EGrAMS, provide documentation of current interoperability capabilities, respond to all required funding and compliance assurances, and adhere to all reporting and fiscal requirements. Indirect cost and cost allocation documentation may be required (see linked instructions document).
