The grantee will serve as the subrecipient to establish a baseline that can be used to establish targets and demonstrate CHW-related outcomes. The funds will support coordinated outreach, data collection, analysis, and reporting necessary to produce a reliable workforce inventory and needs assessment, ensuring efficient use of resources while avoiding duplication of existing efforts.
Eligible applicants include rural health organizations, nonprofits, federally qualified health centers, local health departments, and tribal health entities operating in Michigan. Applicants must demonstrate capacity for interoperability and connectivity initiatives, and meet MI EGrAMS eligibility requirements as outlined in the official application form.
