Synopsis As part of RHTP’s Transforming Rural Health through Partnerships Initiative, the Collaborative Care Integration and Sustainability Fund aims to reimagine Michigan's rural health landscape by testing and scaling new, community-driven approaches to care. This funding opportunity will support innovative rural health efforts that expand access to care and improve population health outcomes across the lifespan. Targeted strategies will focus on enhancing the health and wellbeing of children, adolescents, and families, including initiatives that strengthen behavioral health and prenatal care through cross sector partnerships.
Eligible applicants include Michigan-based public and private nonprofit agencies, health departments, federally qualified health centers, school-based or school-linked health programs, and other organizations serving rural communities. See the Application Form for full eligibility details.
