Michigan Rural Health Closer to Home Hub and Spoke Model - 2027

Summary

As part of RHTP's Care Closer to Home Blueprint Initiative, the Rural Health Care Delivery Hub and Spoke Model aims to transform access to care in rural communities by creating sustainable, community-based pathways that bring health services closer to where people live and work. By connecting partners across health and social sectors, the initiative reimagines care delivery, improves transportation access, and simplifies access to primary and behavioral health. Through innovative approaches to chronic disease management, maternal and child health, and behavioral health, it seeks to improve outcomes while making care more efficient, coordinated, and person-centered. This funding opportunity will support collaborations between healthcare providers and community-based organizations to strengthen outpatient care, improve health outcomes, reduce hospital admissions, and lower healthcare costs.

Eligibility

Eligible applicants include Michigan-based rural health care providers, Federally Qualified Health Centers (FQHCs), rural hospitals, tribal health organizations, community-based organizations with experience in health services, and local health departments. Applicant organizations must demonstrate the capacity to coordinate services within rural communities and form partnerships across health and social sectors.

Requirements Checklist

• Applicants must submit a completed Application Form, a project narrative, detailed budget and justification, organizational chart, and evidence of partnership agreements. Adherence to Indirect Costs and Cost Allocation Plan Instructions is required. Compliance with the Standard Agreement Language provided by MDHHS is mandatory. Projects should align with the initiative's objectives to improve rural health care coordination and access.
