Requirements Checklist

• The selected vendor must:
• Employ or contract with NH-licensed advanced clinical pharmacists to conduct statewide comprehensive medication reviews and produce MSRs.
• Develop, implement, and maintain electronic systems for secure exchange and integration of Medicaid and Medicare pharmacy claims data from PBMs, MCOs, and NH-DMS.
• Utilize technology solutions and validated algorithms to identify high-risk medication scenarios (polypharmacy, ADEs, MRPs, opioid/behavioral health risks).
• Facilitate provider/MCO engagement, workflows, and incentive strategies to promote review and adoption of MSRs and interventions.
• Deliver training and technical assistance to providers, and develop community-based health literacy and medication safety programs.
• Guarantee and report measurable reductions in per-member/per-month (PMPM) costs for Medicaid Managed Care, validated by actuarial attestation.
• Complete required program, fiscal, and evaluation reporting, and participate in program evaluation activities led by State and CMS partners.
• Adhere to all terms, allowable/unallowable costs, and reporting requirements as detailed in the RFP and CMS guidance.
