The program, supported by CMS/HHS, aims to modernize and strengthen care in Maryland’s 18 state-designated rural areas through subawards for innovative projects that bolster sustainable healthcare access, workforce development, and community health. Funding emphasizes potential for impact on access, affordability, and innovative models consistent with CMS core principles. RHTP is supported by a $168,180,837.61 federal award from CMS/HHS, with 100% federal funding, and is implemented in five-year renewable budget periods (2025–2030). Projects must align with one or more of the program's three pillars: rural health workforce transformation, sustainable access & innovative healthcare, and empowering healthy food systems.
Eligible applicants include Maryland Local Health Departments and their regional partners serving state-designated rural areas. Additional eligibility requirements may be specified in the competitive procurement process.
