the Rural Health Transformation Program, targeting rural, frontier, and underserved communities in Idaho. Up to $2,000,000 in federal funding is available ($1,000,000–$2,000,000 per award), with a period of performance from 09/30/2026 to 09/30/2027. Awardees will serve as pass-through organizations issuing and managing subgrants to local partners delivering National Diabetes Prevention Program (DPP) and Diabetes Self-Management Education and Support (DSMES) services across Idaho. Eligible applicants include Idaho-based health systems, healthcare organizations, public health districts, universities/academic institutions, nonprofits, community-based organizations, professional associations, pharmacy organizations/networks, tribal organizations, and other qualified public or private entities with a minimum of two years' experience managing federal or state-funded health programs. Applicants must have an active SAM.gov registration and UEI, and be free of exclusions. There is no cost share or matching required. Indirect and administrative costs are capped at 10% of the total approved budget (calculated on MTDC). A detailed line-item budget and justification are required. Applications must be submitted to IdahoRHTP@dhw.idaho.gov; late submissions will not be reviewed.
Eligible applicants are Idaho-based entities including health systems, healthcare organizations, public health districts, universities/academic institutions, nonprofit and community-based organizations, professional associations, pharmacy organizations/networks, tribal organizations, and other qualified public or private entities. Applicants must have a minimum of two years' experience managing federal or state-funded health programs, possess active SAM.gov registration and UEI, and not be debarred or excluded from federal programs.
