Solicitation No. RHTPDIS1 (Federal Award No. RHTCMS332084, ALN 93.798), addressing RHTP Initiative 4. Total available funding $2,000,000, anticipated 1-2 awards, award range $1,000,000-$2,000,000. Eligible applicants: state/local government agencies, tribal organizations, 501(c)(3) nonprofits, Idaho institutions of higher education, healthcare associations/professional societies, Idaho hospitals/health systems, FQHCs, organizations with statewide clinical-education/workforce-development experience. No cost share/match required. Lead Entities will implement evidence-based chronic disease prevention and cancer screening initiatives for rural/frontier Idaho, including work plans, technology dashboards, community events, and regular reporting. Administrative and indirect cost limit: 10% of total approved budget, on MTDC base. Full detailed, line-item budget and justification required per 2 CFR 200 Subpart E. Subgrantees may serve as pass-throughs for local partners.
