the Rural Health Transformation Program (RHTP) in West Virginia. The purpose is to restore workforce participation by improving health and employment outcomes for adults with chronic, non-behavioral physical health conditions—including diabetes, cardiovascular disease, respiratory disease, musculoskeletal conditions, and other prevalent chronic disorders—that act as barriers to employment. Funded programs must deploy dedicated RTW liaisons and supervisors for individualized participant engagement, field-based coordination, and employer partnership, including work-health transition planning and job retention support. Applicants are required to provide evidence of operational readiness within 60 days of award, proactive participant intake, scalable employer engagement, and robust data and reporting integration with the State’s RHTP data spine. Funding is performance-based; payment is contingent on achieving verified milestones: operational launch (within 60 days), participant intake and engagement, job placement, employment verification, 180-day employment retention, and demonstration of scalability and sustainability. Direct clinical care or billable medical services are explicitly excluded. Proposed models must avoid payer duplication, demonstrate prior RTW coordination experience, operational/governance capacity, and sustainable strategies beyond the grant period. Only non-employer organizations (such as nonprofits, health systems/provider-affiliated entities, workforce organizations, or universities) may apply; employers, however, must be formal implementation partners and commit in writing. Projects that target SUD or primary behavioral health barriers as their focus are not eligible under this funding (covered under other mechanisms). The AFA aligns with State and CMS reporting requirements, and all grantee data will be considered a State asset for oversight and analytics.
Eligible applicants are nonprofit organizations, health systems or provider-affiliated entities, state universities or colleges, workforce or employment-support organizations, and regional/community-based organizations with demonstrated employer engagement in work-health programs. Applicants must demonstrate (1) operational and governance structure to deploy field-based RTW staff; (2) ability to coordinate across healthcare and employer settings; (3) compliance with State and CMS reporting and technical standards; (4) non-duplication with reimbursable clinical and payer services; and (5) sustainability approaches to support long-term operations post-grant. Employers are ineligible as direct grant recipients but must be named as implementation partners, with actionable roles and defined populations in commitment letters. Organizations seeking to focus primarily on SUD or mental health RTW services are ineligible—these areas are funded separately.
